l\u2019\u00e9tablissement compte tenu des situations sp\u00e9cifiques.<\/p>\n<p>Il assure et d\u00e9finit :<\/p>\n<ul>\n<li>La mise en place d\u2019outils d\u2019identification des \u00e9l\u00e8ves, de diagnostic des situations et de suivi des \u00e9l\u00e8ves pris en charge (apport d\u2019indicateurs, d\u2019informations permettant d\u2019\u00e9clairer la situation du jeune\u2026).<\/li>\n<li>La proc\u00e9dure de recueil et de transmission des informations.<\/li>\n<li>L\u2019accompagnement \u00e0 la d\u00e9finition de projets scolaires ou professionnels.<\/li>\n<li>L\u2019organisation et le suivi du devenir scolaire ou professionnel des \u00e9l\u00e8ves sortis r\u00e9cemment de l\u2019\u00e9tablissement.<\/li>\n<li>La mise en place et le suivi des parcours ou d\u2019actions de remobilisation et de rem\u00e9diation.<\/li>\n<\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section d\u2019actions men\u00e9es ;<\/li>\n<li>Nombre de parcours personnalis\u00e9s \u00e9labor\u00e9s&#8230;.<\/li>\n<\/ul>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section
